COVID-19 SCREENING CHECKLIST

Employees are required to self-screen prior to the start of every shift at a TRCA office, site or facility.

This checklist is meant to keep you and your colleagues healthy and safe. It is only a basic checklist and is not meant to take the place of medical advice, diagnosis, or treatment. We are asking that before entering any TRCA facility or site that you complete the following self-screening tool:

Q: Have you been diagnosed or determined to have a probable case of COVID-19 for which you have not been medically cleared to return to work?

Q: Do you or anyone that you have close contact with have any of the following symptoms: fever, new or worsening cough, new or worsening running nose or nasal congestion, difficulty breathing (shortness of breath), or a combination of these symptoms?

No

Q: Have you had close contact with anyone with a confirmed or probable COVID-19 case within the last 14 days?

Yes

No

Q: Have you or anyone that you’ve been in close contact with travelled outside of the country and has returned in the last 14 days?

Yes

No

IF YOU HAVE ANSWERED YES TO ANY OF THESE QUESTIONS, DO NOT ENTER THE FACILITY. CALL YOUR SUPERVISOR AND HEALTH CARE PROVIDER FOR NEXT STEPS. YOUR SUPERVISOR WILL CONTACT HUMAN RESOURCES AND WILL BE FOLLOWING UP WITH YOU.

Any employee that experiences COVID-19 symptoms is required to notify their supervisor immediately and arrange to leave the facility in a safe and timely manner.
