Report an Incident
This page provides a single first point of contact for reporting workplace incidents, and near misses, of various kinds.
Staff must report any occurrence which caused or may have caused a loss in the course of work. This includes, but is not limited to:
- property damage,
- motor vehicle incidents,
- equipment damage,
- security breach,
- financial loss,
- business interruption,
- reputational damage,
- injury; or
- environmental spills
The dropdown tab below can be used to select the correct incident report or system.
First select the type of incident that has occurred. Once you have done so, a link will appear below that will take you to the appropriate reporting system for the incident at hand.
Please note that some of the links will direct you to IBM notes databases. In order to use IBM Notes databases remotely, you will need to be signed in to TRCA networks using Dell NetExtender Virtual Private Network (VPN). Step by step instructions how to use VPN access can be found here.
