COVID-19 Screening Update

Good morning,

The mandatory COVID-19 Daily Screening Form located on the Staff Hub, as well as the Visitor/Contractor online screening form have been to reflect changes issued by Ontario Ministry of Health on January 31, 2022.

The following questions have been added/updated:

• In the last 5 days have you experienced any of the symptoms below? (if immunocompromised it is in the last 10 days)
• Fever and/or chill
• Cough or barking cough (croup)
• Shortness of breath
• Decrease or loss of smell or taste
• Muscle aches/joint pain
• Extreme tiredness
• Sore throat
• Runny or stuffy/congested nose
• Headache
• Nausea, vomiting and/or diarrhea
• In the last 5 days, have you tested positive for COVID-19?
• Do any of the following apply?
• You live with someone who is currently isolating because of a positive COVID-19 test.
• You live with someone who is currently isolating because of COVID-19 symptoms.
• You live with someone who is waiting for COVID-19 test results.
• In the last 5 days, have you been identified as a “close contact” of someone who currently has COVID-19 and doesn’t live with you?

Please ensure you are completing the COVID-19 Daily Screening Form each day you are attending a TRCA office, site, or facility. The screening is required to be completed prior to you commencing work. If your screening indicates DO NOT PROCEED, please refrain from enter the office/site/facility and contact your supervisor immediately.
