COVID- 19 Screening Update

Good morning,

The mandatory COVID-19 Daily Screening Form located on the Staff Hub, as well as the Visitor/Contractor online screening form have been to reflect changes issued by Ontario Ministry of Health on December 30, 2021.

The following questions have been added/updated:

Are you currently experiencing any of these issues? Call 911 if you are.

  • Severe difficulty breathing
  • Severe chest pain
  • Feeling confused or unsure of where you are
  • Losing consciousness

Are you currently experiencing ONE or more of the symptoms below that are new or worsening? (symptoms should not be chronic or related to other known causes or conditions)

  • Fever and/or chills
  • Cough or barking cough (croup)
  • Shortness of breath
  • Decrease or loss of smell or taste

Are you currently experiencing TWO or more of the symptoms below that are new or worsening? (symptoms should not be chronic or related to other known causes or conditions)

  • Muscle aches/joint pain
  • Extreme tiredness
  • Sore throat
  • Runny or stuffy/congested nose
  • Headache
  • Nausea, vomiting and/or diarrhea
  • In the last 10 days, have you been identified as a “close contact” of someone who currently has COVID-19 (confirmed by PCR or rapid antigen test)?
  • In the last 10 days, have you received a COVID Alert exposure notification on your cell phone?
  • In the last 10 days, have you travelled outside of Canada?
  • In the last 10 days, has anyone you live with been sick with symptoms associated with COVID-19 and/or tested positive for COVID-19 (on a rapid antigen test or PCR test)?

Please ensure you are completing the COVID-19 Daily Screening Form each day you are attending a TRCA office, site, or facility. The screening is required to be completed prior to you commencing work. If your screening indicates DO NOT PROCEED, please refrain from enter the office/site/facility and contact your supervisor immediately.
